一例狂犬病免疫球蛋白注射發(fā)生皮膚過敏報道
藥物過敏反應(yīng)在臨床時有發(fā)生，而免疫球蛋白藥典未報道過有過敏現(xiàn)象發(fā)生。5月19日，在急診注射室出現(xiàn)了一例狂犬病人免疫球蛋白注射引起的過敏反應(yīng)，這是我院自開展狂犬病人免疫球蛋白注射6000多例以來的第一例。當(dāng)天， 10歲女童陳寶寶被狗抓傷胸部由家人陪同來我院外科門診就診，女童胸部有約5厘米長的抓痕，屬于三級暴露，遵醫(yī)囑給予注射狂犬病人免疫球蛋白，因?yàn)閯┝看螅?/FONT>9ml）醫(yī)囑予分次注射，執(zhí)行護(hù)士詳細(xì)詢問患兒無過敏史、未空腹后首次肌肉注射劑量的一半，約兩分鐘左右，護(hù)士巡視中發(fā)現(xiàn)患兒胸前出現(xiàn)紅色皮疹，詢問患兒主訴局部皮膚有發(fā)熱感，無氣喘紫紺等，體溫36.7℃，生命體征監(jiān)測正常，匯報值班醫(yī)師囑繼續(xù)加強(qiáng)觀察，半小時后皮疹漸消退，遵醫(yī)囑予剩余劑量的一半（2ml）肌肉注射后患兒訴注射側(cè)面部麻木，上身再次出現(xiàn)皮疹，其它無不適，醫(yī)囑停止余量注射，觀察三十分鐘后所有癥狀消失離院。狂犬病人免疫球蛋白為血制品，注射前不需要做皮試，一般無不良反應(yīng)，少數(shù)人在注射部位會有紅腫、疼痛感，無需特殊處理，可自行消失。該病人出現(xiàn)的癥狀自我院開展此項(xiàng)治療以來尚屬首例，雖然未引起嚴(yán)重不良后果，卻再次提醒大家，在治療護(hù)理過程中病情觀察不可忽視，護(hù)理無小事，安全是大事。（門急診吳艷萍）
